It seems logical that an American, in America, would go to a hospital in America for urgent heart care.

No matter what you have heard, your insurance status does not determine whether you get life-saving emergency care. It never has.

You get the care without regard to insurance or payment. The issue then, after the fact, becomes who will pay for it. That is where private insurance comes into play.

No one is being denied needed care because of inability to pay or lack of insurance. The liberals sold the country of "X number of people have no healthcare".

It was, and has always been, a lie.

The other lie is that having insurance gets you health care and makes you healthier.
Insurance does you no good if you don't go to the doctor, have a reasonably healthy diet, engage in preventative care, and stop or reduce your use of illegal drugs.

Hospitals are legally bound to provide service until you are stabilized. In many cases, they will go beyond that and provide free care until such time it is either not needed or you check out. I can tell you many stories about "no-pay" individuals receiving care above and beyond anyone's expectations.
