I don't know myself, but I would suggest asking your dealer for it.

They cash the check in Atlanta, then forward the forms to West Virginia. The check cashing is at the very beginning of the process.

If you look at the "ticker" in Scutter01's signature in this thread, (or many, many other threads for that matter) you'll see how long he's been waiting on a tax stamp for his suppressor since they cashed his check.

I think you have her home phone number, the 304 number is her work phone.

Also, before you call, they'll need the serial # of the suppressor to look up the paperwork.

I was told they can't do it with a last name.

Yeah, since the election things certainly are "normal", they're just a little bit backlogged right now. They're so backlogged that they're not even cashing the checks quickly anymore.
